Survey is not Triggering

Hi Experts,

I have to create a same Survey on 3 different table 1) Requested Item 2) Service Request and 3) Catalog Task. I need to trigger survey on "My Assessment and Survey" page then user click on "Take Survey" then Question should display.So i have created a survey with 3 trigger conditions on different tables. So it is working for only Service Request table not for others.

Also i have tried 3 different surveys as well but not working. Could any of you please share any feedback on this.

Can you share your trigger conditions? For anyone to be able to help we need more information.

If you have a condition that the user not receive a survey more than every x number of days that could be why you only get the survey firing from one of the 3 tables, since the user was already presented that survey.

Also what is the business case of firing a survey to the user from the Request, the Request Item and the Catalog Task? Seems a bit excessive if it is the same survey each time?
