Unable to update a catalog items Icon or Picture

So this is a new one for me....We are on London

I did an Insert and Stay on one of our existing catalog items (which I have done 50+ times probably) and now I cannot update the Icon or Picture on the new item (the [Update] and [Delete] options do not exist):

Anyone ever seen this?
