take survey widget is not working for me

when I am using take_survey widget it is showing invalid URL. Can some one help why it is showing. How to configure processor.

Hi,

The survey link only works for the assigned to user (unless it's a public survey -- which is a different service portal link instead of: "take_survey"). So if you're testing a survey...is it assigned to you?

If you visit in the standard UI: Survey > Ready to take, do you see your name as the assigned to? If not, it's only accessible by that person.
